Before you are declared bankrupt, you will need to complete a bankruptcy form which can be obtained free of charge, from either the local court that deals with bankruptcy or on-line.
For county court, you will need the completed forms and two copies of each before your petition for bankruptcy is accepted. For petitions at the High Court, you won't need any extra copies. When you have completed and printed both bankruptcy forms, signed and dated the bottom of every page, and have the required fees, you can ask for your petition to be dealt with at court. The bankruptcy forms required are:
- Insolvency Rules 1986 form 6.27. This is the petition and is your request to the Court to be made bankrupt
- Insolvency Rules 1986 form 6.28. This is called the statement of affair and outlines all your assets, your debts, creditors and the amounts owed. You must also sign a declaration of insolvency. This is a sworn oath made in the presence of an officer of the court or a solicitor
